What is Penile Prosthesis Implantation?
Penile prosthesis implantation is a definitive and effective treatment for men with erectile dysfunction (ED) who have not responded to other therapies such as oral medications, penile injections, or psychological counseling.
For decades, this procedure has been considered a safe and reliable solution to restore sexual function and improve quality of life.

Types of Penile Prostheses

  1. Semi-Rigid Prosthesis:
  • Consists of two bendable rods implanted inside the penis.
  • The penis remains in a semi-rigid state and can be positioned upward or downward manually.
  • Advantages: ease of use, long durability, and lower cost.
  1. Inflatable (Hydraulic) Prosthesis
  • Includes two inflatable cylinders inserted into the penis, a pump placed inside the scrotum, and a fluid reservoir implanted in the abdomen.
  • By pressing the pump, fluid is transferred into the cylinders, creating an erection.
  • In the flaccid state, the penis appears natural and soft, making it more discreet compared to the semi-rigid type.

Indications for Penile Prosthesis

  • Severe, persistent erectile dysfunction unresponsive to medication.
  • Erectile dysfunction following prostate or pelvic surgery.
  • Peyronie’s disease with severe penile curvature.
  • Neurological conditions (e.g., spinal cord injury, advanced diabetes).
  • Dissatisfaction with other ED treatments.

Surgical Procedure

  • Performed under general anesthesia or spinal anesthesia.
  • A small incision is usually made either below the abdomen or between the base of the penis and scrotum.
  • The prosthesis is inserted into the corpora cavernosa of the penis.
  • For inflatable prostheses, the pump is placed in the scrotum and the fluid reservoir in the abdomen.
  • Surgery typically lasts 1–2 hours.

Possible Complications

Although uncommon, potential risks include:

  • Infection at the surgical site
  • Temporary or persistent pain
  • Device malfunction or mechanical failure
  • Decreased penile sensation or discomfort during intercourse
  • Need for revision surgery or device replacement

Postoperative Care

  • Take antibiotics and pain relievers as prescribed.
  • Keep the surgical site clean and change dressings during the first few days.
  • Avoid sexual activity until cleared by your doctor (usually 4–6 weeks).
  • Patients with inflatable prostheses will be trained on how to use the pump.
  • Attend regular follow-up visits to assess healing and device function.

Preoperative Instructions

  • Shower and shave the genital and abdominal area.
  • Stop taking blood thinners such as Aspirin, Warfarin, or Heparin.
  • Fast for at least 8 hours before surgery.
  • Remove all jewelry, watches, and accessories before entering the operating room.
  • Do not wear glasses, dentures, hearing aids, contact lenses, or prosthetics.
  • Empty the bladder immediately before surgery.
  • Inform your doctor and nurse about smoking or substance use, and avoid smoking for at least 24 hours prior to surgery.
