Gender Selection via IUI
The principle behind gender selection using IUI is based on separating sperm that carry the X chromosome from those carrying the Y chromosome.
How the Procedure Works:
- The woman first undergoes ovulation induction with medication. Her ovarian response is monitored through ultrasound to assess follicular growth and maturity.
- Once the appropriate time is determined, the IUI procedure is scheduled.
- On the day of the procedure, the couple visits the clinic together. A sperm sample is collected from the male partner.
- In the lab, the sperm is processed using techniques such as Swim-Up, Albumin Gradient, or Percoll Gradient—methods that separate sperm based on motility or density. These methods help isolate either X- or Y-bearing sperm.
- Based on the couple’s desired gender, the selected sperm sample is then introduced into the woman’s uterus using a specialized catheter.
Eligibility Criteria for Women:
To be eligible for gender selection via IUI, the woman must:
- Be ovulating during the treatment cycle.
- Have at least one open and functional fallopian tube
- Receive an adequate number of motile sperm
- Be free of any active uterine or pelvic infections
Sperm Collection Guidelines:
IUI is typically performed with fresh sperm, collected on the same day as the procedure. To ensure optimal sample quality:
- 1 The male partner should abstain from ejaculation for at least 2 days and no more than 7 days prior to sample collection.
- 2 The sample must be collected via masturbation, not intercourse.
- 3 It should be collected in a sterile container provided by the clinic to avoid contamination.
- 4 Lubricants or gels should not be used, as most of them are toxic to sperm.
The Insemination Procedure:
- The patient removes clothing from the waist down, lies on an examination table, and places her feet in stirrups.
- A full bladder is not required for all patients.
- A speculum is inserted to visualize the cervix, which is then cleaned.
- The processed sperm is inserted into the uterus using a thin catheter passed through the cervical canal.
- After insemination, the patient should remain lying on her back for about 10 minutes. While studies show no significant difference in pregnancy rates between those who rest and those who don’t, the short rest may provide emotional reassurance.
Post-IUI Gender Selection Care:
- Normal daily activities can be resumed shortly after the procedure.
- A wet sensation post-IUI is due to cervical mucus discharge, not sperm leakage.
- Mild abdominal discomfort or cramping may occur. If needed, acetaminophen can be used for pain relief.
- Light spotting may occur and is not a cause for concern.
- The couple should avoid intercourse for one week following the procedure.
- A pregnancy blood test (β-hCG) should be performed two weeks after the procedure.
- Some studies suggest that vaginal progesterone supplementation in stimulated IUI cycles may increase pregnancy rates, but this is subject to your physician’s recommendation.
Possible Risks and Complications:
- Upper genital tract infection: Though rare, infection is a potential complication.
- Ectopic pregnancy: It is not yet clear whether IUI increases the risk of ectopic pregnancy compared to spontaneous pregnancies in women with similar risk factors.
- Ovarian Hyperstimulation Syndrome (OHSS): This may occur in cycles where ovulation-stimulating drugs and HCG are used.
- Multiple pregnancies: The risk of twins or higher-order multiples increases, especially in cycles stimulated with injectable medications. However, the risk is lower when Clomiphene citrate is used alone.
