Treatment Process for Couples Undergoing IVF or ICSI

Initial IPD Admission
- Upon arrival at the International Patient Department (IPD), the IPD officer reviews the couple’s medical documents, creates a medical file, provides consultation, and requests a new semen analysis for the male partner.
- The couple is then referred to the IPD physician.

Initial Medical Evaluation
- The IPD physician screens the couple for infectious diseases and completes the International Health Record (IHR) form.
- If the male partner is eligible to provide a semen sample (after 2–7 days of abstinence), the couple is guided to the hotel-based collection rooms.

Specialist Consultations
- The couple, accompanied by the IPD officer, visits the fertility and gynecology specialist.
- If infertility is identified as male-related, the husband is referred (with IPD assistance) to the urologist for priority consultation and treatment.
- If no male factor is found, the female partner’s treatment process begins with a consultation and ultrasound (ideally 4–5 days before the menstrual cycle).

Ovarian Stimulation
- Based on the gynecologist’s assessment, ovarian stimulation medications are prescribed to start on day 2 or 3 of the menstrual cycle.
- The patient must return to the clinic at scheduled intervals (every 5–6 days, 3 days, 2 days, or even daily, depending on the physician’s decision) for ultrasound monitoring of follicular growth.

Pre-Operative Preparation
- Once follicular maturity is confirmed by ultrasound, the physician prescribes the trigger injection and sets the date for oocyte retrieval.
- On the same day, the patient completes pre-surgical procedures:
- General physician consultation
- Pre-operative laboratory tests
- Surgical file registration
- Payment of surgical fees

Oocyte Retrieval (Puncture Procedure)
- On retrieval day, if the male partner can provide a semen sample, the couple is directed to the collection rooms. The sperm sample is processed in the laboratory for fertilization.
- The female partner, accompanied by the IPD officer, is admitted to the Royesh Surgery Center.
- At admission, patient data is registered, and initial evaluations by the IPD nurse and IPD physician are documented.
- The oocyte retrieval procedure is performed under general anesthesia in the operating room.

Post-Operative Care
- After the procedure, the patient is transferred to the international patient ward under IPD nurse supervision.
- The patient is closely monitored, particularly for signs of Ovarian Hyperstimulation Syndrome (OHSS).
- If stable, discharge instructions are provided.
- Patients with no OHSS and who do not wish for fresh embryo transfer may return to their home country 2–3 days after retrieval.

OHSS Management
- If OHSS develops, fresh embryo transfer is not possible. All embryos will be frozen, and the patient must remain in Iran for 7–10 days after retrieval for monitoring, ultrasound, and any necessary treatment.

Embryo Transfer
- If the patient is stable and wishes to proceed with fresh embryo transfer, the procedure is scheduled 3–5 days after retrieval, depending on embryo development stage.
- The IPD officer coordinates admission and referral to Royesh Surgery Center.

Embryo Freezing
- If patients opt for embryo freezing:
- A consent form is completed.
- After payment, they may return to their home country.
- Embryo transfer can be planned at least 1 month later.
- Embryo storage can also be renewed online from abroad.

Post-Transfer Options
- After embryo transfer, patients may either:
- Return to their home country immediately, or
- Stay in Iran for up to 14 days until the pregnancy test.
Additional Notes:
- The female treatment journey typically requires at least 1 month.
- A patient handbook is available, containing care instructions and additional medical services offered at the center.
